You must complete this form for any birds you plan to show at the fair. You will need to create a separate form for each child entering. You may use the same email address and phone number, but each account must be created under the individual child’s name. YES—we know some of you have a lot of kids and/or lots of birds! Thank you for your patience as you fill these out.

DEADLINE: January 31, 2026

If the form will not allow you to add more birds, go ahead and submit it, then complete an additional form for the remaining entries.

Important: Even if birds will be shown as a pen of three (such as laying hens or Cornish Cross meat birds), each bird must be entered individually on the form and must have its own leg band number listed.

Please list every bird you might bring. This information will be used to confirm ownership and leg band numbers at fair and check-in. We strongly recommend registering all birds that have even a potential to end up at fair now to avoid eligibility issues later. It is also best to register birds under each child’s name in case birds need to be switched between siblings prior to fair.

Don’t forget to include any Cornish Cross meat birds or quail that will be ordered or hatched in the coming months. Even though their hatch dates fall after the ownership deadline, they must still be declared as owned.
